Your Primary Care Physician (PCP)

If you are an HMO member your primary care physician (PCP) is responsible for managing and coordinating your care, and when appropriate, referring you to Lakeside's network of specialists and hospitals in your community. Each Lakeside member, who has an HMO insurance plan, chooses his/her own PCP who will provide all primary care services including routine office visits, health evaluations, immunizations and other routine procedures.

Your PCP will coordinate all of your medical care. After regular hours, your PCP can be reached through their answering service or their staff will provide necessary instructions. For routine matters, it is best to call during scheduled office hours.

When selecting a PCP, please remember that anyone over age 18 must select either a family medicine, general practice or internal medicine physician. Most family medicine physicians will also treat pediatric patients.

Your health plan may require an office visit copay which is to be paid at the time of your appointment.
