

We see patients by appointment only. To make an appointment, call the office during business hours, and schedule your visit as far in advance as possible.
When making your first appointment, please have your insurance card available and be prepared to clarify the reason for seeing the physician. At the time of your first visit bring your insurance card, all of your current prescriptions and medical information or previous medical records; we can request these for you. The physician will review your health history and then assess any health problems.
Bring your insurance card to each appointment and notify us of any change of address or insurance plan.
Please arrive on time. If you are delayed, call our office to check the doctor’s availability or to reschedule. To cancel an appointment, call our office at least 24 hours in advance.
Co-payments or payments that are the responsibility of the patient are to be collected when service is provided. We accept cash, personal check, and VISA or MasterCard. Please discuss any questions about payment, billing, reimbursement, insurance plans, etc., with the office manager prior to your visit.
Your physician may determine that your medical condition requires a referral to a specialist. Based on the requirements of your insurance plan, we will complete the necessary paperwork and refer you to the appropriate specialist. Many insurance plans allow us to directly refer patients to a specialist without having to wait for prior approval. We verify the physician network for HMO patients.
For all other types of insurance, which require a referral, confirm with their office that their services are covered by your insurance. Most of the specialists we refer to are affiliated with most major insurance plans.
